Nat Burkey Tattoo Request Form
Books are OPEN for Nat Burkey. If your tattoo design is chosen, you will receive an EMAIL with information about the next step.
It may take multiple days for me to review all of the applications so please be patient in regards to hearing back from me!
A few important bullet points to read:
Filling out an appointment request form does NOT guarantee that you will get an appointment in this booking round.
A deposit is required to schedule your tattoo appointment if picked.
All tattoo appointment requests MUST be submitted through my website using the “Nat’s Appointment Request” form.
My rate is $180 per hour.
If you must cancel or reschedule for ANY reason, it must be done at least 48 hours prior to your appointment day or you will be asked to place another deposit to reschedule.
If you plan on getting tattooed with someone, each person has to fill out their own separate request form.
I will not be doing any cover-ups.
Please only submit for one design.
I unfortunately cannot accept everyone who submits a form. I can only accept a very limited amount of projects at a time so keep trying!
